The example would be to add to an item with tokens for Universal Saves to +3. In addition to add Fortitude saves of +3 as well? Would this result in Fortitude saves to +6 on that item.

As a secondary question what is the max that you can add to saves? Is it +12 like attributes, +5 like skills, some other , unlimited or something else?

The most you can add with the token system is +3 to saves. I am not sure if you added +3 universal twice if it would stack, and I am not sure how typed saves and universal on the same item stack

If you have bonuses on all saving throws they stack up to +20.
I'm not quite sure how it works with general and specific saves but it probably can be found here: http://nwn.wikia.com/wiki/Saving_throw

I don't know about on a single item, however +Universal stacks with +Fort/Reflex/Will. Enough of my low levels have taken advantage of that to fight low level water elementals (drown is always DC20 regardless of size)

Update: DL gave me an idea so I opened up the tools in a test mod I have.

Universal and fort do not stack on the same item. And for the token cost difference for universal vs individual save I would just stick with the universal.

It does stack if on different items though and tested and 20 is indeed the limit.
